Membership Fee Schedule (as of 11/03/2003 )

First year members only:	$100
Renewing Members :
Annual Medicaid Waiver Revenues $250,000 and under.	$150
Annual Medicaid Waiver Revenues greater than $250,000 and under $500,000	$300
Annual Medicaid Waiver Revenues greater than $500,000 and under $1,000,000	$500
Annual Medicaid Waiver Revenues greater than $1,000,000 and under $2,000,000	$700
Annual Medicaid Waiver Revenues greater than $2,000,000	$1,000

Charter/Supporting Members may give additional amounts above the designated levels.

Annual membership dues are billed every January. Payment must be made in one lump sum within 30 days of invoice. Charter/Supporting memberships above the member levels can be paid over a period of time as agreed upon by the HWPA board.
